- Misdiagnosed angina for acid reflux disease resulting in myocardial infarction and death.
- Liver laceration to a patient with TTP (thrombocytopenic purpura) during laparascopic cholecystectomy (gallbladder removal) surgery resulting in death. TTP is a bleeding disorder.
- Untimely diagnosis of cauda equine syndrome , an impingement of the nerves enervating the bladder, bowel, sexual organs, resulting in loss of function , partial paralysis and a dropped foot.
- Excessive radiation during radiation treatment to patient with multiple myeloma resulting in radiation pneumonitis, respiratory failure and death.
- Misdiagnosed brain aneurysm that ruptured resulting in visual and motor deficits.
- Hip replacement surgery on the wrong hip , resulting in surgery on both hips. Patient never walked again and died in a nursing home within one year.
- Lack of informed consent for PTMR (laser of the heart ) during angioplasty procedure, resulting in hole to heart and open heart surgery.
- Misdiagnosis of PID infection , pelvic inflammatory disease, resulting in infertility.
- Negligent removal of endometriotic tissue resulting in partial resection of colon.
- 10 cm. laceration to infants cheek during C-section.
- Hip replacement surgery resulting in foot drop and paralysis.
- Infected knee replacement , resulting in multiple surgeries and fusion of knee.
- Sexual assault during OB/GYN exam.
- Misdiagnosed diverticulitis resulting in rupture, peritonitis, reversible colostomy.
- Transection of lingual and alveolar nerves during third molar (wisdom teeth) surgery. Resulting in loss of taste, and loss of sensation to tongue, cheek and mouth.
- Misdiagnosis of infection of diabetic foot, resulting in loss of toes.
